El delirio como forma de restitución dentro de la estructura psicótica

La presente tesina aborda los conceptos de psicosis y delirio, tanto desde la psiquiatría como desde el psicoanálisis, profundizando los abordajes realizados desde esta última orientación. La investigación está organizada en tres partes de desarrollo teórico y una última parte en donde se aplican los conceptos a un caso clínico. En la primera parte, se realiza un recorrido que permite observar, a lo largo de la historia, el lugar que se ha ido otorgando a los trastornos mentales y sus manifestaciones. Luego, se abordan los conceptos mencionados desde los principales referentes de la psiquiatría, mostrando las concepciones y clasificaciones más antiguas y, también, aquellas que forman la semiología psiquiátrica actual como son la CIE 10 y el DSM IV. En la segunda y tercera parte, se establece el marco teórico psicoanalítico. Desde la teoría freudiana, se trabaja principalmente con aquellas obras en las que estudia la psicosis, especialmente la paranoia, y el mecanismo de formación de los fenómenos psicóticos. Luego, se aborda el delirio en la psicosis, como un “parche” con el cual el sujeto cubre la desgarradura producida por la perturbación de los vínculos entre el yo y el mundo exterior. Por último, se toman los textos en los que Freud entiende la construcción delirante como favorecedora de un orden ante la experiencia de desestabilización vivida en la psicosis. Desde la teoría lacaniana, se toman conceptos de esta enseñanza para explicar cómo se estructura el sujeto psicótico, y el lugar que puede tener en dicha estructura, la elaboración delirante. Se desarrolla el modo en que ésta última puede funcionar como una suplencia de la metáfora paterna ausente, permitiéndole al sujeto psicótico lograr una estabilización. También se puntualiza el lugar de semblante del objeto “a”, lugar de vacío, que debe tomar el analista en la clínica de la psicosis. En la cuarta y última parte, se analiza el caso del filólogo y escritor Jean-Pierre Brisset a partir de los conceptos psicoanalíticos abordados en el marco teórico.

In the present work the concepts of psychosis and delirium are treated, both from the psychiatry and from the psychoanalysis, deepening the studies realized from this latter theory. The research has been organized in three parts of theoretical development and a last part in which the concepts are applied to a clinical case. The first part presents a study of the place that has been reserved to the mental disorders and their manifestations, along the history. Then, the mentioned concepts are considered from the main paradigms of the psychiatry, showing the most ancient conceptions and classifications as well as those that constitute the current psychiatric semiology, as in the CIE 10 and the DSM IV. In the second and third part, the theoretical psychoanalytic frame is developed. From the Freudian theory, the works that have been taken into account are those in which Freud studies the psychosis, specially the paranoia, and the mechanism of configuration of the psychotic phenomena. Then, the delirium is analyzed in connection to the psychosis, as a "patch" with which the subject covers the fracture produced by the disturbance of the links between the ego and the outside world. Finally, the texts in which Freud takes the delirious construction as one that allows an order before the experience of destabilization lived in the psychosis, are also considered. Some concepts of the Lacan’s theory have been also integrated in order to explain how the psychotic subject is structured, and the place that might have the delirious elaboration in the psychotic structure. The way in which this delirious elaboration might function as a substitute of the absent paternal metaphor, allowing the psychotic subject to achieve a stabilization is also developed. The place of semblance of the object "a", a place of emptiness, which must be taken by the analyst in the clinic of the psychosis, has been also specified. In the fourth and last part, the case of the philologist and writer Jean-Pierre Brisset is analyzed applying the psychoanalytic concepts exposed in the theoretical frame.
